Steps Counseling Center offers downloadable copies of our new client forms. These forms may be downloaded and printed at your convenience to bring with you on your first visit to our office. We are unable to accept these forms electronically or via fax due to privacy laws.
When arriving for your first appointment it is suggested you arrive 5-10 min. early to complete any additional paperwork that maybe required by your insurance company or employer.
If you are planning to utilize your insurance benefits- please bring your insurance card with you so we can verify benefits.
